2) Not sure which of two doctors wrote the prescription?
3) Don't have the patient's Medicaid ID at hand?
4) Aaaaaaand you're trying to give authorization for multiple controlled drugs, including "Oxycontin and Oxycodone" (which makes as much sense as telling someone to take Tylenol and Acetaminophen )
5) And you say you need to find out what mg the doctor wanted?
